Steps for Admission into Morgan Estates
- Fill out and complete the admissions application and return to the Admission Coordinator for review
- Pre-admission nurse screening completed by Case Manager
- Schedule medical appointment for doctor to complete required medical evaluation form (forms are only valid for 30 days once completed)
- Make copies of appropriate insurance cards, social security cards, health care proxy, power of attorney and any other important forms you may want us to have on file
Once Morgan Estates has received the above items, our team will review and confirm that we have all the appropriate information to proceed. Once approved a move-in date/room will be scheduled by our Admissions Coordinator. Please don’t hesitate to contact our team anytime with questions or concerns regarding the admission process.

New York Connects Livingston County
New York Connects is a community resource for seniors and their families in regards to long term care. It provides a directory of services ranging from assisted living to case management.
The Alzheimer’s Association is a valuable resource for information on care, symptoms, activities and support.
